Signup: Developmental Skills Check

Thank you for your interest in signing your student for an occupational therapy (OT) Developmental Skills Check with Hands in Motion OT! These evaluations take place at your child's daycare, preschool, or private school. The recommendations and observations made during the OT evaluation will be sent to the email address below within one week followed by a 10 minute phone call to discuss the results. The cost of these low-complexity evaluations are $35.

I give permission for my child to participate in an occupational therapy (OT) Developmental Skills Check offered by Hands in Motion OT, LLC. I understand that this low-complexity OT evaluation will be scheduled to take place by a licensed occupational therapist during my child’s school day. I understand that the evaluation may consist of a combination of classroom observation, teacher input, and/or direct student participation in developmental activities for a holistic screening perspective. I understand that in signing up for an evaluation, observations are shared with my student's teacher/the program director to best support their development, growth, and independence. I agree to list any medical diagnoses or allergies that are relevant to treatment. I understand payment of $35 will be collected through Zelle at the time of service.
